Each evaluation includes a comprehensive online mental health evaluation with a licensed provider, review of relevant information, assessment of symptoms and functional limitations, and completion of appropriate workplace documentation when clinically supported.
Our goal is to make the process easier for individuals navigating mental health-related leave, short-term disability, FMLA/CFRA, and workplace accommodation requests.

Initial Evaluation — $159
Includes:
- 30-minute tele-health focused mental health evaluation
- Comprehensive review of your symptoms and concerns
- Review of relevant medical, mental health, and work history
- Review of completed screening questionnaires (PHQ-9, GAD-7, and LEAPS)
- Clinical assessment of how your symptoms affect your ability to work
- Review of supporting documentation you provide, if applicable
- Completion of initial certification documentation, if clinically appropriate
- Completion of documentation within 24 hours after all required information has been received
- Secure electronic delivery of completed documentation through the patient portal
Documentation is provided only when clinically supported and is based on the evaluating clinician's independent professional judgment.
Follow-Up Evaluation — $159
A follow-up evaluation is required for requests related to a certification previously completed by Tymly, including:
- Extensions of leave beyond the initial certification period
- Certification for a new certification period
- Updated disability, FMLA/CFRA, or workplace accommodation documentation related to the original evaluation
- Return-to-work forms, clearance letters, or updated work-status documentation
- Reassessment of symptoms and functional limitations
Includes:
- Follow-up telehealth evaluation
- Review of your current symptoms and functional status
- Review of any new or updated information
- Clinical assessment to determine whether continued leave, return to work, or updated documentation is medically appropriate
- Completion of updated documentation, if clinically appropriate
- Documentation completed within 24 hours after all required information has been received
- Secure electronic delivery of completed documentation through the patient portal
Follow-up evaluations are available only for certifications previously completed by Tymly. Requests to extend, modify, or assume responsibility for documentation issued by another healthcare provider require a new Initial Mental Health Evaluation. Return-to-work documentation also requires a follow-up evaluation. All recommendations and documentation are based on the clinician's independent medical judgment and are not guaranteed.
