1. Payment Responsibility
Tymly provides mental health evaluation services through the applicable clinical entity based on the state where services are provided.
For individuals located in California, clinical services are provided by Layne Nursing PC.
For individuals located in Louisiana, clinical services are provided by Tymly LLC.
Tymly is a private-pay service and does not bill insurance companies directly. Patients are responsible for payment for all services provided.
Payment is due prior to the scheduled appointment unless other arrangements have been approved.
2. Services and Fees
Fees for Tymly services are disclosed before services are provided.
Fees may include:

Initial mental health evaluations

Follow-up evaluations
-
Clinical reassessment
-
Review of relevant records
-
Documentation completed as part of an applicable evaluation

Other clinical services disclosed before they are provided
Payment covers the clinician's professional time and evaluation services.
Payment does not guarantee:

A diagnosis
-
Completion of requested documentation
-
Recommendation for workplace leave
-
Disability certification
-
Workplace accommodation recommendation
-
Return-to-work clearance
-
Approval of FMLA, CFRA, or other workplace leave
-
Approval of short-term disability benefits
-
Approval of benefits or requests by an employer, insurer, leave administrator, government agency, or other third party
All diagnoses, clinical recommendations, documentation decisions, and other clinical determinations are based on the evaluating clinician's independent professional judgment.
3. Flat-Fee Appointments
Unless otherwise disclosed before scheduling, Tymly uses a flat-fee appointment model.
The current fee for an individual mental health evaluation is disclosed on the Tymly website and during scheduling.
The applicable appointment fee covers the evaluation and documentation associated with that evaluation when documentation is clinically appropriate.
A separate follow-up evaluation may be required for:

Extensions of leave
-
Recertification
-
A new certification period
-
Updated disability documentation
-
Changes in work status
-
Return-to-work documentation
-
Updated workplace accommodation requests
-
Other documentation requiring a current clinical reassessment
Patients will be informed of any applicable fee before a follow-up or additional service is provided.
4. Insurance and Superbills
Tymly does not participate with insurance plans and does not submit claims directly to insurance companies.
Upon request, the applicable clinical entity may provide a superbill containing available billing information that you may submit to your insurance company for possible out-of-network reimbursement.
Patients are responsible for contacting their insurance company to determine:

Whether out-of-network mental health benefits are available

Whether the services received are eligible for reimbursement

Applicable deductible or coinsurance requirements

Required documentation or claim-submission procedures
-
The amount of any potential reimbursement
Tymly, Layne Nursing PC, and Tymly LLC do not guarantee insurance reimbursement and are not responsible for claim decisions, denials, reimbursement amounts, processing delays, or other decisions made by an insurance company.
5. Additional Services and Documentation Requests
Additional services requested after an evaluation may require a follow-up appointment or additional fee.
Examples may include:

Clinical reassessment

Review of newly submitted records
-
Documentation for a new certification period
-
Extension or recertification requests
-
Updated forms
-
Return-to-work documentation
-
Updated workplace accommodation documentation

Material revisions requiring additional clinical assessment
Administrative corrections that do not require additional clinical review may not require a new appointment.
Whether a new evaluation is clinically necessary is determined by the evaluating clinician.
Patients will be informed of applicable fees before additional services are provided.
6. Refund Policy
Fees are charged for professional healthcare services provided by the clinician.
Refunds are not provided for services that have already been performed, including:

Completed evaluations

Clinical assessments
-
Clinical review
-
Review of submitted records
-
Documentation services completed in connection with an evaluation
A refund is not owed solely because:

Requested documentation is not clinically supported

A clinician does not recommend leave or disability
-
A third party denies a patient's request
-
An employer does not approve leave
-
An insurer denies disability benefits
-
A leave administrator requests additional information
-
A patient disagrees with the clinician's professional assessment
If an appointment is canceled, missed, or rescheduled, any refund or fee will be determined according to Tymly's Cancellation and No-Show Policy and applicable law.
7. Payment Processing
Payment may be collected through a third-party payment processor.
The clinical entity responsible for the patient's services may vary by state.
For California services, payment for professional clinical services is associated with Layne Nursing PC.
For Louisiana services, payment for professional clinical services is associated with Tymly LLC.
Patients may receive invoices, receipts, or other payment documentation identifying the applicable clinical entity responsible for the services provided.
8. Payment Does Not Guarantee Documentation
Payment is for the clinician's professional evaluation and time.
Payment does not purchase or guarantee:

A medical certification
-
A disability determination
-
A particular diagnosis
-
A specific duration of leave
-
Completion of a requested form
-
Approval of an accommodation
-
Return-to-work clearance
-
Approval from an employer, insurer, government program, or other third party
Documentation is completed only when the evaluating clinician determines that it is clinically appropriate and supported by the evaluation.
9. Patient Responsibility
By scheduling and paying for services, you acknowledge that:

You understand Tymly is a private-pay service

You are financially responsible for services received
-
Insurance reimbursement is not guaranteed
-
Payment does not guarantee a particular clinical outcome
-
Follow-up evaluations require additional payment
-
Third parties make their own decisions regarding leave, disability benefits, accommodations, and other requests
-
You are responsible for providing complete and accurate information relevant to your evaluation
